In order to create a GHCMyChart ℠ account, you will need a unique activation code. Before we can assign your activation code, we must verify your identity. If you are over the age of 18, please complete the form below with your information.

Once you’ve completed the form, it may take up to three calendar days for processing and verification. Once approved, you will receive an email with your activation code and instructions on how to activate your GHCMyChart℠ account.

Parents or guardians with an active GHCMyChart℠ account may submit a FamilyChart Request for patients under the age of 18. The parent or guardian must be logged in to their own account to do this
